Abstract     Antigens of ABO histoblood group system are the most important transplantation antigens in solid organ transplantations. The reason for their importance is two-fold: ABO antigens are expressed on almost all cells in the body and in the serum we have antibodies against ABO antigens that are absent in the individual's own tissues, which can induce a hyperacute rejection of grafts expressing foreign A/B antigens and haemolysis of incompatible red blood cells. Regarding the ABO blood group of the recepient and the donor, the liver transplantations can be ABO-identical, ABO-compatible or ABO-incompatible. In Slovenia the majority of liver transplanted so far have been ABO-identical and the rest have been ABO-compatible. In some days following the ABO-compatible liver transplantation the incompatible antibodies anti-ABO can occur in the serum of the recepient, which can cause immune haemolysis of the red blood cells. These ABO-antibodies originate from donor lymphocytes, which are transfered with the transplanted organ. For the recepient and the liver donor ABO, RhD blood group and indirect antiglobulin (Coombs) test, for detection of irregular erytrocyte antibodies other than ABO, must be determined. For the operation the ordered number of RBC units, with ABO blood group of the recepient or ABO compatible with his serum, are crossmatched. In order to prevent possible haemolytic anemia following ABO-compatible liver transplantation, the recepients must be transfused with RBC units, that are ABO-compatible with the sera of the recepient and the donor, untill the anti-ABO incompatible antibodies disappear. Antibodies titers and direct antiglobulin (Coombs) test are also determined in such recepients.
