Abstract     Outcome of orthotopic liver transplantation in patients with hepatitis B virus related chronic liver disease depends on prevention of allograft infection. Longterm prophylaxis with hepatitis B immunoglobulin has dramatically lowered the rate of reinfection and increased patient survival. Lamivudin treatment befor transplantation and combination prophylaxis with lamivudin and HBIG post transplantation is currently the most effective strategy to prevent recurrence of HBV infection in HBV replicative patients with cirrosis. Future resarch should test new protocols using lower HBIG doses given intravenous or intramuscular alone or in combination with antiviral agents.
