Abstract     The renal functional abnormalities and ascites formation in cirrhosis are the final consequence of circulatory dysfunction. There is no evidence to support a role of reduced vascular oncotic pressure due to hypoalbumunemia in their pathogenesis. The available clinical evidence does not support the use of albumin infusions for such indications. In contrast, albumin infusions are very effective in preventing the deterioration in renal function associated with large volume paracentesis or spontaneous bacterial peritonitis. Concomitant administration of albumin and vasoconstrictor drugs normalises almost completely circulatory function and improves renal function in patients with hepatorenal syndrome.
