Abstract     The metabolic syndrome is a constellation of risk factors of metabolic origin that are accompanied by an increased risk for cardiovascular disease and type 2 diabetes. These risk factors are atherogenic dyslipidemia, elevated blood pressure, elevated plasma glucose, a prothrombotic and a proinflammatory state. The two major underlying risk factors for the metabolic syndrome are obesity and insulin resistance. The IDF consensus criteria for the diagnosis of the metabolic syndrome set the increased waist circumference, a measure of the abdominal obesity, as the leading criterion, while the diagnosis is made when 2 out of 4 risk factors are presentin addition: elevated triglycerides, blood pressure and glucose, and decreased HDL level. The condition is progressive, exacerbating factors are physical inactivity, advancing age, and endocrine and genetic factors. In many patients the condition culminates in type 2 diabetes that further increases the risk for cardiovascular disease. Primary treatment of the metabolic syndrome is a lifestyle therapy-weight loss, increased physical activity, and anti-atherogenic diet. Drug therapies directed toward the individual risk factors might be required; the decision for their use is made according to the guidelines of the cardiovascular risk management.
