Abstract     Activation of the renin-angiotensin-aldosterone and sympathetic nervous systems in patients with heart failure contributes to maintenance of perfusion of vital organs. However, chronic activation leads to maladaptive consequences with negative effects. It has been shown that drugs that decrease the activation of the neurohormonal systems significantly reduce symptoms and signs of heart Failure and improve the prognosis. Several large clinical studies confirmed the effectiveness of treatment with angiotensin converting enzyme inhibitors, angiotensin receptor blockers, beta blockers and aldosterone antagonists in patients with heart failure. In spite of this improvement, patients with heart fallure still have poor prognosis. We need more data about optimal dosages and combinations of different drugs and the effectiveness of new pharmacological and nonpharmacological treatments.
