Abstract     Anticoagulant treatment increases the risk of bleeding during invasive procedures, while interruption of treatment increases risk of thromboembolism. Therefore, has to be carefully managed to avoid periprocedural complications. Recommendations for periprocedural management of anticoagulant therapy are given for elective as well as urgent procedures according to periprocedural bleeding risk and the patient's thromboembolic risk. Minor procedures are performed without interruption of anticoagulant treatment. For major procedures coumarins are interrupted and in patients with very high thromboembolic risk bridging therapy with low molecular heparin is instituted.
