Abstract     Contrast nephropathy is the third most frequent cause of acute renal failure in the hospital. It develops more often in patients with preexisting renal failure, in patients with heart failure, diabetes mellitus, hypotension and plasmocytoma. Before every contrast procedure the risk assessment for predicting contrast induced nephropathy is appropriate to be done. For the procedure the patient must be well hydrated, parenteral hydration with 0,9% saline or isotonic Na bicarbonate 12 hours before and after the procedure is recommended at speed 1 ml/kg hourly. Prevention with various drugs, as with N-acetylcysteine, is quite controversial. The volume of contrast agent must be as small as possible.
