Abstract     Despite major improvements in antiplatelet therapy and technique of stenting, thrombotic events remain the primary cause of death after percutaneous coronary interventions. Subacute stent thrombosis occurs up to 30 days after stent procedure. It usually results in ST-segment elevation myocardial infarction or sudden death. Presence of residual dissections, long stents, plaque burden, small calibre of stented vessel, bifurcation lesions, low ejection fraction, renal failure and diabetes were recognised as predictors of subacute thrombosis. The premature discontinuation of clopidogrel is associated with an 30-fold grater risk of subacute thrombosis and remain the most important reason. The present data confirm the need to educate referring physicians and patients about the consequences of deviating from prescribed antiplatelet regimen.
