Abstract     A patient with diffuse pulmonary infiltrates accompanied by respiratory insufficiency is a diagnostic and therapeutic challenge for a doctor. A patient may be afflicted by acute infilitrates of undetermined aetiology or patient's disease may have enhanced or there are some complications. In cases of fluminal course it is necessary to diagnose the basic disease and the source of the acute respiratory insufficiency deterioration as soon as possible so as to start the aetiological treaunent. Beside diseases which can be often found in the intensive care, for example cardiogenous lung endema, acute respiratory distress syndrome (ARDS) and both-sided pneumonia, diffuse interstitial lung diseases (DIPB) belong to this group as well. Some of them respond to imunosupressive treatment, others do not, which means they are in a terminal phase. Diseases in this phase must be determined as such so we can treat patients paliatively without the invasive approach. The focus of this paper is the diagnostic lung treatment and DIPB, which can be treated speeifically.
