Abstract     Transient left ventricular apical ballooning presents with clinical and electrocardiographic features of acute myocardial infarction and can provoke severe left ventricular failure in patients with normal coronary arteries. The emotional stress with increased catecholamine release is the most probable etiology of the syndrome which is most frequently described in Japan. We report a case of a 74-year-old female, who was admitted an cardiogenic shock, due to transient apical akinesia. Coronary angiography finding was normal and no myocardial necrosis was confirmed, but the nuclear investigation with Tc 99m sestamibi showed reversible left ventricular anterior wall perfusion defect. The recovery was complete with no symptoms or signs of coronary artery disease even three years after acute event.
