Abstract     Achilles tendon is the largest and most powerful tendon in the body, presenting the insertion of triceps surae muscle to the os calcis. Acute ruptures of the Achilles tendon are relatively common injuries with the increasing incidence, what could be associated with the changing life style. Its etiology is multifactorial, and its true nature remains unclear. The best management method of the acute Achilles tendon rupture is controversial and debated. Operative complications could be avoided with the use of conservative treatment that is associated but with a significantly higher re-rupture rate with diminished functional results. We can restore the original length with the open surgical repair, what brings to a low re-rupture rate with the quick and successful return of the high number operated on patients to the preinjuries activities. It is associated but with a significantly greater risk of particularly major Cutaneous suturing, particularly under local anasthesie, significantly reduces the risk of major complications, but can be associated with the higher (although statistically insignificant) numbers of aural nerve disturbances. With the use of (biomechanically stronger) percutaneous method comparable functional results to the open repair can be managed, with the early mobilisation and comparable re-rupture rate.
