Abstract     Osteoporosis is the most common systemic skeletal disease. It has been identified as a silent orthopaedic "epidemic" because the fracture is usually the first sign of the disease. Because of the fractures it is already one of the major health care problems of the developed countries. In the paper fundamentals of the biomechanics of osteoporotic fractures, diagnostic procedures and treatment are presented. The new paradigm of diagnostic and treatment is emphasized. The bone mineral density (BMD) is just one of the risk factors for the osteoporotic fracture and the term bone strength is coming to the fore. All orthopaedic surgeons who treat middle aged and elderly patients with fractures have a duty to think about, and institute mechanisms for preventing, ,future fractures if possible.
