Abstract     Spontaneous bleedings are frequent in patients with severe haemophilia. 85% of all bleeding events occur in joints. Recurrent haemarthroses lead to chronic synovitis, synovial hypertrophy and progressive degeneration of the joint - haemophilic arthropathy. Radiosynoviorthesis is a method of treatment of synovitis or synovial processes by intra-articular injection of beta emitting radionuclides. It can be used also in patients with haemophilia suffering from frequent spontaneous bleedings in joints. The aims of our study were to evaluate the effectiveness of the treatment by radiosynoviorthesis in haemophiliacs and to assess the effective dose of unwanted radiation that haemophiliacs receive after treatment with 186Re-colloid. The effectiveness of radiosynoviorthesis was evaluated in 28 haemophiliacs in whom 40 treatments were performed between January 2001 and July 2004. In 14 treatments with 186Re-colloid assessment of the effective dose was done. In the year after radiosynoviorthesis at least 50% decrease in frequency of spontaneous bleeding events in the treated joint was detected in 62.5% of treatments. At least 50% decrease in coagulation factors consumption to prevent the bleedings in the treated joint was established after 52.5% of treatments. Both, decrease in bleeding frequency and decrease in coagulation factor consumption, were statistically significant (p < 0.005). Majority of patients were satisfied with the effects of radiosynoviorthesis in the year after treatment. The effective dose of unwanted radiation received by haemophiliacs, treated with 186Re-colloid, was 28 ± 9 microSv/MBq of activity injected in the joint. We conclude that radiosynoviorthesis decreases spontaneous bleeding events in patients with haemophilia and that radiosynoviorthesis with 186Re-colloid is a safe treatment method.
