Abstract     Four fixation techniques for a fibrinogen and thrombin coated collagen fleece, used as a scaffold in the cartilage repair, were compared simulating the initial post-operative period in the knee joints on seven human cadaveric inferior extremities An oval full-thickness chondral lesion, size 2.5 cm2, was made on the central part of the medial femoral condyle. Four scaffolds without seeded chondrocytes were implanted into each lesion using four fixation techniques consecutively: self-adhesion without additional material (SA), fibrin sealant (FS), bone sutures (BS), or periosteal cover (PC). The operative time, costs of fixation material, and the skin incision length were acquired. After each implantation 150 cycles of continuous passive motion (CPM) were performed. The scaffolds were evaluated after every 30 cycles through re-opening of the central part of the arthrotomy. The fixation strength was tested after the motion was completed. Two cases were additionally exposed to 50 cycles of 10 and 20 kg loading after the CPM was completed. I'he fastest and cheapest was the SA technique, whereas the opposite holds true for the PC technique that also required additional exposure for periosteal harvesting. All the SA scaffolds were detached before 60 cycles of CPM. The other scaffolds remained stable throughout the testing with only minor disruptions. The endpoint fixation strength was higher Tor BS and PC than for the FS scaffolds. The FS scaffolds were detached as a result of additional load cycles, while the BS and PC scaffolds showed substantial cicformations. SA technique of tested scaffold did not provide sufficient fixation. The FS fixation technique was easy to perform and assured satisfactory scaffold stability. (Abstract truncated at 2000 characters)
