Abstract		Destructive processes associated with periodontal disease lead to loss of periodontal tissues. Ideally surgical treatment of periodontitis should be aimed not only at preventing further loss of connective tissue and alveolar bone but also at regeneration of the tooth supporting apparatus. The latter may be achieved with various surgical techniques: flap procedures, placement of bone or bone substiture grafts, or use of organic or synthetic barrier membranes. In a motivated patient, the dentist's goal is to preserve natural teeth, observing the principles of a systematic approach to the treatment of periodontal disease. Dental implants have proved a successful solution for a large proportion of partially dentate or fully edentulous patients. However, an implant is only a substitute for a lost tooth. It is important to remember that in a patient who is not adequately motivated for oral hygiene maintenance, inflammation and destruction of alveolar bone can occur also around an implant.
Summary		Procesi razgradnje med parodontalno boleznijo vodijo v izgubo obzobnih tkiv. Idealno kirurško zdravljenje parodontalne bolezni naj bi ne le preprečilo nadaljnjo izgubo vezivnega prirastišča in čeljustne kosti, ampak doseglo obnovo obzobnih tkiv. Obnovo obzobnih tkiv poskušamo doseči z različnimi kirurškimi tehnikami: s premiki režnjev, z vstavitvijo kosti ali kostnih nadomestkov, z uporabo organskih in sintetičnih membran in z dodajanjem bioloških modulatorjev. Pri motiviranem pacientu v skladu z načeli sistematičnega pristopa k zdravljenju parodontalne bolezni poskušamo ohraniti pacientove lastne zobe. Zobni vsadki, ki so v visokem odstotku dokazano uspešna rešitev za paciente z delno ohranjenim zobovjem ali pa za brezzobe paciente, so samo nadomestek za izgubljene zobe. Vedeti moramo, da se pri pacientih, ki niso dovolj motivirani za vzdrževanje ustrezne ustne higiene, pojavi vnetje in propad čeljustne kosti tudi ob vsadku.
