Abstract     BACKGROUND. For years placebo has been used in day-to-day medical treatment, but its effects have only recently been subject of research. Some of the studies which have examined the placebo effect confirni that compared to standard, this type of treatment is very successful. Treatment of focal ciystonia is an area of research where the affects of placebo have not yet been studied. AIM and HYPOTHESIS. The aim of the study was to determine whether the number of patients who react to placebo treatment is the same as the number of patients who react to botulinum treatment, and to define which variables influence the placebo response. METHODS. We conducted a randomised double-blind crossover study, which comprised of 39 patients with torticollis/blepharospasm. After defining the placebo reactors, we made a case-control study. We then described the variables with descriptive statistics and calculated the differences between cases and controls using ttest and chi-square test. The probability that a variable will influence the placebo response was calcuated using unconditional univariate and multivariate logistic analysis. RESULTS. Using two different definitions we identified 56% placebo reactors. The patients reported that the initial improvement after both injections (placebo and botulinum) occured after 5,9 days. We observed a statistically significant difference between placebo and botulinum injections for the duration of the drug effect, percentage of improvement and side effects (tduration = 4,21, p <= 0,001; timprovement = 4,68, p <= 0,001; tside effects = 2,08, p = 0,04).
