Abstract     In the reconstructions after extensive ablative surgeryfor head and neck carcinomas microvascular free flaps have shown several ndzvantages over pedicledflaps, particularly in preoperatively irradiated patients, and therefore they represent the method ofchoice in this type of surgery. During the years 1985-1990, microvascular free flaps were used in 38 patients treated at the University Department of Otorhinolaryngology and Cervico-Facial Surgery in Ljubljana; of these,10 had recurrent carcinoma of the facial skin,18 carcinoma of the oral cavity and oropharynx, and 10 subglottic and/or tracheal stenosis. Reconstruction of the mandible was performed in 5 patients with carcinoma of the floor of the mouth, in two cases by means of an osteocutaneous flap of the iliac crest, whereas in another two a radial forearm flap and in one case a scapular flap were used. For reconstruction of the skin or mucosa radinl forearm flaps were used most frequently, but apart from these, upper forearm flap and latissimus dorsi flap were used as well. Metatarsal osteocutaneousflap was used in 3 patients with tracheal stenosisfor the reconstruction of the external wall, in 7 cases of laryngo-tracheal stenoses the epitheliazed microvascular iliac crest flap was applied for the reconstruction of the external wall. As to the duration of surgery, hospitalization time and the rate of complications, our results are comparable with those reported in the literature.
