Abstract     Prosthetic joint infections, although rare complications are associated with significant morbidity and high costs. They can be classified as early or delayed, thay can also be of hematogenous origin. Various therapies can be used in the treatment, considering the duration of clinical symptoms, stability of the implant, susceptibility of the isolated organism to antimicrobial agents and patient's comorbidities. The authors present 33 patients treated for infected prosthetic joints at the Orthopaedic department of the Teaching hospital Maribor in the 8 years period. Coagulase negative staphylococci were identified as a cause of infection in half of the treated patients, Staphylococcus aureus was isolated in 12 % of patients, followed by diphteroids, streptococci, enterococci ect. Fifty five percent of patients were treated for early infection with implant retention, 30 % of patients had delayed infection of prostethic joint. 15 % of patients suffered from hematogenous infection of the implant. Patients were successfully treated with different treatment strategies in over 90 %.
Summary     Okužbe endoprotez so v ortopediji redke, vendar so povezane z veliko obolevnostjo, močno podražijo in podaljšajo zdravljenje. Razdelimo jih, glede na čas nastanka, na zgodnje okužbe, odložene okužbe in hematogene okužbe, katere lahko nastanejo kadarkoli po vstavitvi proteze. Pri zdravljenju vnetja protez se odloča, glede na trajanje okužbe, stanje proteze, povzročitelja vnetja in druge spremljajoče dejavnike, za različno obsežne posege. Na Ortopedskem oddelku Splošne bolnišnice Maribor so v 8 letnem obdobju (1999-2006) obravnavali 33 bolnikov z vnetjem endoprotez. V polovici primerov so bili povzročitelji vnetja koagulozno negativni stafilokoki, v 12 % Staphylococcus aureus, redkeje pa difteroidi, streptokoki in drugi. Petinpetdeset odstotkov bolnikov je imelo zgodnjo okužbo endoproteze in so jih zdravili z ohranitvijo proteze, 15 % bolnikov je imelo akutno hematogeno okužbo, 30 % bolnikov odloženo okužbo proteze. Z različnimi načini zdravljenja so uspešno zdravili 90 % bolnikov.
