Abstract     Sudden emotional stress can trigger stress cardiomyopathy with acute, reversible left ventricular apical akinesia and severe systolic dysfunction, in which sympatho-adrenergic stimulation seems to play the central role. Exercise-induced myocardial injury and dysfunction during strenuous exercise has been reported in hunting dogs, and exerciseinduced cardiac fatigue in humans. Although they are both induced by different types of stress (emotional stress vs. physical stress), the two entities are related to excessive sympatho-adrenergic stimulation and should have similar myocardial effects. The author analyses the pathogenetic mechanisms of stress cardiomyopathy, and speculates on the existence of analogous "exercise cardiomyopathy" and its pathogenetic mechanisms. In the author's opinion, stress cardiomyopathy and exercise-induced cardiac fatigue represent analogous entities and suggest the name exercise cardiomyopathy instead of a rather lay term cardiac fatigue. Finally, the diagnosis of stress cardiomyopathy was not recognised until 2005, although the folk expression "broken heart" had been used for centuries.
