Abstract     Pertusis syndrome is respiratory tract infection which is characterised with typical attacks of spasmodic cough, followed by whoops. In most cases Bordetella pertussis is causing organism, but clinically indistinguishable disease may be caused by other bacteria (Bordetella parapertussis, Bordetella bronchiseptica, Chlamidia trachomatis) and viruses (adenoviruses, respiratory sinciicial virus, influenza, parainfluenza, citomegalovirus). To determine the influence of therapy with midecamicin on the convulsive stage of whooping cough, we studied the clinical course of pertusis in 37 patients, who were randomised in treated group ( 18 patients) and control group (19 patients). We found that there was no statisticaly significant difference in the number of paroxisms daily between treated and control group and we conclude that therapy with midecamicin, started in late prodromal or early paroxismal stage of disease, has no influence on clinical course of whooping cough, but shortens the period of contagiousness of patients with B. pertusis infection. In addition we found that nasopharygeal swab is, despite low sensitivity, the best test for determination of etiology of pertusiform cough, while available serologic methods, because of unpredictable serologic reaction of organism, represent only additional diagnostic help.
