Abstract     Background: The triage of orthodontic patients is frequently based on subjective professional judgement, although a number of methods have been developed for objective assessment of the severity of malocclusion. In the present study, two such methods, the EFO (Eismann-Farčnik-0vsenik) index and IOTN index (index of orthodontic treatment need), were compared in terms of reliablity and time needed for each assessment. Material and methods: The study was conducted on 100 casts of the permanent dentition from 100 patients (53 female and 47 male, average age 15.4 years). Reliability of each method was examined by measuring intra-examiner agreement, inter-examiner agreement and agreement wtih the Eismann-Farčnik (EF) index, which was used as the gold standard. Results were analysed using the intra-class correlation coefficient (ICC). The time needed for individual assessments was recorded. Results: The IOTN method yielded significantly higher severity ratings than the EFO method. Agreement with the EF index was excellent for the EFO method (ICC = 0.930) and satisfactory for the IOTN method (ICC =0.572). Intra-and inter-examiner agreement was good for both methods (ICC > 0.750). Individual assessments with the EFO index took on average 5.5 minutes, while those with the IOTN method took only 2.0 minutes. Conclusions: The EFO occlusive index and the IOTN are reliable and simple methods for estimating the severity of malocclusion. The EFO index shows better agreement with the standard method, probably because it evaluates a greater number of functional traits.
