Abstract     Veuous thrombosis (VT) is an important cause of morbidity and mortality. Our approach to the diagnosis of VT has evolved over the years. If the clinical diagnosis is unreliable, donors can combine the results of pretestprobability, D dimer tests and compression ultrasound imaging for a safe and convenient strategy in suspected lower extremity thrombosis. Patients with a low pretest probability and a negative D dimer Best result can have proximal VT excluded without the need for diagnostic imaging. The mainstay of treatment of VT is anticoagulation therapy. The use of low molecular weight heparin enables outpatient management for most patients with VT. Decisions regarding the optimal duration of anticoagulation to prevent recurrent VT should be individualised and the risk of recurrence should be balanced against the risk of bleeding and the inconvenience of continuing treatment.
