Abstract     Pyogenic liver abscess and echinoccocosis are rare infections of liver parenchyma. Ultrasonography and ultrasonographically guided aspiration of the abscess, followed by microbiological investigations of the aspirate are crucial in the diagnosis of the pyogenic liver abscess. However, if echirroccocal infection is suspected, serological investigations are the method of first choice. Diagnostic aspiration of echinoccocal cyst is relatively contraindicated because of the possibility of anaphilactic reaction due to the release of the echinoccocal cyst content. A case of a 51-year-old patient with clinical picture, laboratory investigations and imaging, all suggesting pyogenic liver abscess, is presented. Serological screening test on Echinococcus granulosus, which was done prior to the diagnostic aspiration of the presumed abscess to exclude echinoccocal infection, was positive in low titres. 'Che confirmatory Westernblot test on Echinococcus granulosus (using specific antigens) was negative. Having consulted a parasitologist, we concluded that the puncture of the lesion is indicated and safe. However, cytological examination of the punctate showed echinoccocal fragments, indicating that our patient had secondary infection of the echinococcal cyst.
