Abstract     Background Continuous monitoring of drug consumption is an important strategy for prudent and cost-effective use of drugs. Antimicrobials are among the most prescribed drugs in outpatient practice and in hospital care. In most cases antimicrobials are improperly prescribed or are even misused. Irrational use of antimicrobials is clinically ineffective and leads to higher treatment costs. Clinical ineffectiveness due to irrational use additionally leads to loss of confidence in antimicrobial drugs, unnecessary exposure of patients and development of antimicrobial resistance. Antimicrobial resistance has become a major global health problem as it presents an imperative for development of new potent antimicrobials which are necessarily associated with markedly higher treatment costs. Material and methods This survey was focused on rational prescribing of antimicrobial drugs. The data on consumption of antimicrobials for various clinical departments of the Jesenice General Hospital were collected by the hospital pharmacy. WHO ATC 2005 classification system, which ranks antimicrobials in a large group J01: drugs for systemic treatment of bacterial infections and Defined Daily Dose as a measuring unit according to the WHO ATC/DDD methodology was used. Antimicrobial use at the Jesenice General Hospital in the period between 1998 and 2004 was estimated by the Department of Infectious Diseases, Medical Centre Ljubljana as a part of European Surveillance on Antibiotic Consumption project (ESAC). Statistical part of survey was performed by the Chair of Biopharmaceutics and Pharmacokinetics, Faculty of Pharmacy, University of Ljubljana. Results After year 2000 a trend of decrease in antimicrobial consumption was observed. (Izvleček prekinjen pri 2000 znakih)
Summary     Izhodišča Spremljanje porabe zdravil je pomemben del dejavnosti, ki so potrebne za zagotavljanje racionalne uporabe zdravil in posledično znižanje stroškov za zdravljenje. Protimikrobna zdravila so med najpogosteje predpisanimi zdravili v bolnišnicah in v ambulantni praksi. Tovrstna zdravila se predpisujejo prepogosto in velikokrat nepotrebno in neustrezno. Neracionalno protimikrobno zdravljenje pa ni samo dražje, ampak je klinično neuspešno in ustvarja nezaupanje v zdravila te vrste, nepotrebno obremenjuje bolnika in pospešuje odpornost mikroorganizmov na protimikrobna zdravila, kar je eden največjih problemov današnje medicine. Posledično je potreben razvoj novih, učinkovitejših protimikrobnih učinkovin, ki pa so vse dražja. Materiali in Podatke o porabi vseh zdravil po oddelkih in celokupno v Splošni bolnišnici Jesenice (SBJ) metode evidentira bolnišnična lekarna. Natančneje smo preučili smotrnost porabe zdravil iz skupine protimikrobnih učinkovin. Uporabili smo klasifikacijski sistem WHO ATC 2005, ki uvršča protimikrobne učinkovine v veliko anatomsko-terapevtsko skupino J01: zdravila za sistemsko zdravljenje bakterijskih okužb in dnevno definiran odmerek (DDD) kot mersko enoto. Podatke o količinski porabi protimikrobnih učinkovin v SBJ v obdobju 1998‐2004 smo pošiljali na Infekcijsko kliniko, kjer so jih analizirali po metodologiji ATC /DDD za potrebe Evropskega projekta spremljanja porabe protimikrobnih učinkovin (ESAC). Podatke smo statistično ovrednotili s pomočjo računalniških orodij Katedre za biofarmacijo in farmakokinetiko na Fakulteti za farmacijo Univerze v Ljubljani. Rezultati Poraba protimikrobnih učinkovin v Splošni bolnišnici Jesenice kaže v preučevanih letih trend upadanja po letu 2000, ki pa ni statistično značilen. Glede na rezultate projekta ESAC za leto 2003 je na splošno v Sloveniji previsoka poraba amoksicilin/klavulanata, kinolonov in 3. generacije cefalosporinov. (Izvleček prekinjen pri 2000 znakih)
