Abstract     Low left to right stroke counts ratio (L/R), obtained at radionuclide gated equilibrium ventriculography, reflects right ventricular volume overload due to tricuspid incompetence. The ratio gives no information on hemodynamic influence of tricuspid regurgitation on venous system and is not accurate in combined left- and right-sided valvular incompetence. This study was performed to assess and quantitate the impact of various degrees of tricuspid regurgitation on liver blood pool during gated equilibrium radionuclide ventriculography. Tricuspid regurgitation was graded with Doppler-echocardiography (DE) according to the regurgitantjet to the right atrium in 25 patients (DE 1 (n=5):člcm; DE 2 (n=11): greather th. 1 less th. 3.5cm; DE 3 (n=9): greather th. 3.5cm). Thirteen patients were investigated with radionuclide ventriculography for other cardiac diseases, had no tricuspid regurgitation and constituted control group (CTR: less th. 0.5cm ). Gated equilibrium ventriculography was performed and left to right stroke counts ralio was calculated. Time-activity curves were generated from right ventricular and liver regions of interest (ROls). A new algorithm was used to compare the liver time-activity curve with right ventricular time-activity curve in every point of cardiac cycle. Changes in liver radioactivity were quantified in relation to right ventricular perforrnance and were expressed as tricuspid regurgitation index (TRI). The influence of regurgitant flow on liver blood pool was graded with visual analysis (vis) of 3). computed liver time-activity curve as absent (vis 0), moderate (vis 1) and pronounced (vis Negative correlation (ro=-0.52, p less th. 0.001) was detected between left to right stroke counts ratio and tricuspid regurgitation index.
