Abstract     Background. GnRH-analogues suppress gonadotropin secretion and consequently affect ovarian steroid production, therefore the effects are obvious in all organs containing sex steroid receptors. The purpose of the research was to establish effectiveness of GnRH-analogue Suprefact in the patients with endometriosis, adenomyosis and myomas. Methods. Eleven patients, aged from 31 to 47 years with myoma uteri (4), adenomyosis (4), habitual abortions (1) and primary infertlity (2) were included in the study. In all the patients enlarged uterus (> 8 cm) was verified by ultrasonography, hysteroscopy, hysterometry and/or laparoscopy. In 8 patients pelvic endometriosis was laparoscopically verified. The tretment with GnRH-analogue (Suprefact, Hoechst) with intranasal application was started on the second day of the menstrual cycle in a daily dose varying from 400 to 1200 micro g according to the side effects and to the indication for therapy. LH, FSH and E2 were determined in the 2nd, 3rd, 4th, 8th, 16th, 20th and 24th week of the treatment and three months after wards. Results. Hormonal analyses revealed that FSH concentration did not change during the treatment period, according to basal values, while LH and E2 concentrations significantly fell in the 4th week of the treatment (from basal Lh 5.55 +- 5.10 IU/l to 1.02 +- 1.06 IU/l in the 24th week - p>0.05; from basal E2 0.90 +- 0.43 nmol/l to 0.10 +- 0.05 nmol/l - p>0.01). Leiomyomas decreased in size for 65 percent and uterine diameter for 21 percent. Those relations were unchanged three months after the treatment, while E2 and LH showed significant increase. In the patients with pelvic endometriosis second laparoscopy showed significant improvement, in all cases for one step (Revised American Fertility Society Classification). Side effects like hot flushes, sweating and vaginal dryness were excessive in the last hree months of the treatment.(trunc.)
