Abstract     We compared the diagnostic value of information given by salivary testosterone with the diagnostic value of information given by total serum testosterone, free serum testosterone, androgen index (FAI = T/SHBG x 100), and sex hormone-binding globulin. To examine the clinical value of these parameters we analyzed blood and saliva samples from 32 hirsute women and 32 age-matched healthy control volunteers. The levels of salivary testosterone, total serum testosterone, free testosterone, and the androgen index were all significantly higher in the hirsute group (p < 0,01), whereas in the sex hormone-binding globulin (SHBG) a decrease was noticed (p < 0,01). Out of 32 hirsute patients salivary testosterone was elevated in 27, total serum testosterone in 7, free testosterone in 20, and the androgen index in 26 patients; in 24 patients a decrease in SHBG was noticed.
