Abstract     Easy availability of testosterone measurement with direct automated assays and debate about testosterone levels which influence prognosis of prostate cancer have prompted many to measure testosterone levels in their prostate cancer patients on adrogen deprivation theraoy. Results are often not as expected (despite claims of LHRH agonists manufacturers, testosterone values are not in castration range according to guidelines). Our results on 115 patients and literature review shed some light on this paradox: present chemiluminiscent assays do not perform well in range of castration values. However, if the test is consistent (as Abbott Architect), it may give some infomation not only on remaining testosterone level, but on overall serum androgen level, which may be useful for predicting prognosis, selecting patients for combined androgen blockage or other treatment decisions.
