Abstract     Rheumatoid arthritis is a complex and heterogeneous, chronic, inflammatory, rheumatic disease. A part of genetic component of the disease is assigned to HLA associations. Population studies are a way of establishing associations of HLA and rheumatoid arthritis. The pathology of rheumatoid arthritis is not fully explained yet, but includes complex interactions of T and B cells, macrophages and fibroblast-like synovial cells and involves many cytokines, which act autocrine and paracrine. Development of specific biological agents, that target soluble or membrane-bound molecules, started a revolution in therapy of rheumatoid arthritis. New therapies also create an opportunity for a better understanding of the pathogenesis of the disease.
Summary     Revmatoidni artritis je kompleksna in heterogena kronična, vnetna revmatična bolezen. Del genetske komponente bolezni pripisujemo povezavi s HLA aleli. Način ugotavljanja povezave med HLA lokusom in revmatoidnim artritisom so bile populacijske študije. Patologija revmatoidnega artritisa še ni povsem pojasnjena, vključuje pa kompleksne interakcije med T in B limfociti, makrofagi in fibroblastom podobnimi sinoviociti ter vpleta mrežo citokinov, ki delujejo avtokrino in parakrino. V zadnjih letih je razvoj bioloških učinkovin, ki specifično ciljajo topne ali na membrano vezane molekule, sprožil revolucijo v terapiji revmatoidnega artritisa. Razvoj novih terapij pa je dal možnosti za boljše razumevanje patogeneze te bolezni.
