Title     Prognostic value of bone marrow neutrophil granulocyte reserve in multiple myeloma

Abstract     Background and methods. In a retrospective study the effect of bone marrow neutrophil granulocyte reserve at diagnosis on survival was evaluated in 75 patients with multiple myeloma. The increase in peripheral blood neutrophil count observed after intravenous administration of 8 mg dexamethasone was used as a measure of bone marrow neutrophil reserve (dexamethasone test, DT). Results. Patients with DT values greather than 2 x 109/L had a better prognosis than patients with lower DT values. Bivariate correlation and multiple regression analyses showed survival in myeloma patients could best be predicted from: a) blood urea concentration, b) rise in neutrophils after dexamethasone, c) percent of plasma cells in bone marrow and d) age. Concluisons. The present results show DT to be a useful parameter for prognostic evaluation of multiple myeloma. Further studies are required to verify its prognostic value, especially in relation to beta2 microglobulin.
