Abstract     Urinary tract infections (UTI) are common pathology in childhood. Despite numerous studies and guidelines in this field there is still no unified, modern, consensus-based algorithm dealing with diagnostic approach to these children. In recent years studies have shown that diagnostic evaluation is probably too extensive, imaging procedures relatively invasive and burdensome for the children, antibiotic prophylaxis not sufficiently rational. In near future, international, based on evidence from appropriate, prospective, long-term studies, indicating which risk children with UTI should be further evaluated and which diagnostic procedures applied, and how to be treated in case of diagnosed urinary tract anomaly, Old problems and new dilemmas concerning childhood UTI are shortly presented in this review article.
Summary     Akutne okužbe sečil so pogosta bolezen otroške dobe. Kljub številnim raziskavam in priporočilom na tem področju, enotnega, sodobnega, mednarodno sprejetega algoritma o obravnavi teh otrok nimamo. V zadnjih letih vedno več raziskav kaže na to, da je diagnostika verjetno prekomerna, preiskave relativno invazivne in za otroka obremenjujoče, antibiotična zaščita premalo racionalna. V prihodnjih letih pričakujemo mednarodno sprejet, z dokazi podprt algoritem o tem, katere otroke po akutni okužbi sečil nadalje diagnostično obravnavati in s katerimi diagnostičnimi metodami, ter kako jih ob ugotovljeni napaki na sečilih zdraviti. V članku predstavljamo stare probleme in aktualne dileme, ki se odpirajo pri obravnavi teh otrok.
