Abstract     Incretin effect makes possible higher secretion of insulin due to release of enterogenic incretine hormones which stimulate secretion from beta cell of pancreas. Incretins are secreted after ingestion and interaction of food with digestive tract. For comparison secretion of insulin after parenteral application of glucose is much lower. On the basis of incretin effect two new group of antidiabetic drugs have been developed. Analogs of glucagon like peptid-1 reach supraphysiological effect of incretins. Inhibitors of dipeptidyl peptidase-4, incretins inactivation enzyme, prolong half time and endogen incretins. Both groups show in clinical trial good effect on lowering blood sugar practically without hypoglycemia. Beside that body weight of patients stay unchanged or even decrease.
Summary     Inkretinski učinek omogoča večje izločanje insulina zaradi sproščanja enterogenih hormonov, inkretinov, ki stimulirajo izločanje beta celic trebušne slinavke. Do sproščanja inkretinov pride po zaužitju in interakciji hrane s prebavili. Za razliko je izločanje insulina pri intravensko aplicirani glukozi manjše. Na podlagi inkretinskega učinka so razvili dve novi skupini zdravil za zdravljenje sladkorne bolezni tipa 2. Analogi glukagonu podobnega peptida - 1 omogočajo suprafiziološke učinke, med tem ko zaviralci encima dipeptidil peptidaze - 4, ki inaktivira inkretine, podaljša razpolovni čas in učinek endogenih inkretinov. V kliničnih raziskavah kažeta obe skupini dober učinek na znižanje krvnega sladkorja in to praktično brez hipoglikemij. Ob tem ostaja teža bolnikov nespremenjena ali se celo zniža.
