Abstract     The sensitivity and specificity ofthe ratio between isoenčyme 1 lactate dehydrogenase rrtd isoen2čme 2 lactate dehydrogenase (LDI/LD2) by the diagnosis of perioperative myocardial infarction (PMI) was established. Twenty-eight patients undergoing coronary artery bypass procedure exclusively were includeded. The results that we have got in the research, were compared with the sensitivity and specificity ofmyocardialfraction ofcreatine kinase (CK-MB). The new scintigraphic method with the antibodies against myosin of ; myocardial muscle (Myoscint Centocor Europe) which were markedfrom 2 to 2.5 mCi (74-93 M Bq) of indium (In111) was used as referent method. Diagnosis of PMI is difficult because there is no classic presentation. Many perturbations of the various potential diagnostic criteria are related not only to possible myocardial necrosis that may arise during the operation but are also influenced by other factors like tissues injury by the operation, ischaemic heart arrest, application of extracorporal circulation, anaesthesia. Reports of PMI frequency vary a lot. Differences in reported frequency of PMI are also partly caused by the difficulty in the diagnosis and dissimilar methods that were used and are not resulted only from different incidence of PMI in different surgical institutions. The distribution of isoenzymes creatine kinase (CK) in the heart and lactate dchydrogenase (LDH) isoenzymes is different. CK isoenzymes are distributed proportionally while LDH isoenzymes are not distributed proportionally. Part of LD 1 in enzymatic activity of after this period LDH in atrium takes 6-7 per cent and rises to 30 per cent in the left ventricular muscle. Part of LD 1 in the left ventricular muscle is the highest. Left ventricular muscle is the only muscle in witch the activity of LD 1 isoenzyme part of LDH enzyme is equal or higher than LD2 part.(trunc.)
