Abstract     Background. Hospital infection within the premises of the Intensive Care Unit is a factor rendering therapeutic endeavours difficult. The lungs are most often affected, particularly in artificially ventilated patients and those with acute resplratory distress syndrome (ARDS). Methods. The majority of tests for the evaluation of such complications are uncertain. We are presenting three cases in which bronchoalveolar lavage (BAL) was introduced in the pulmonary diagnostic procedures in addition to clinical, radiological and ordinary bacteriological methods. In the sample taken for microbiologic analysis the quantitative presence of bacteria and cellular microorganism inclusions were determined. Conclusion. By means of the BAL method a sample from the affected part of the lung for microbiological analysis is taken. In the cases presented, B A L was used in ascertaining the correct diagnosis and in guiding us towards the required antihiotic therapy. The quantitative findings and ascertainment of cellular microorganism inclusions make it possible to discern between colonization and infection.
