Abstract     The latest findings in the use of aspirin in PAD patients from the Aspirin for Asymptomatic Atherosclerosis study - presented in Barcelona ESC 2009 showed clopidogrel as significantly more effective than aspirin in preventing major vascular events in PAD patients, but at a higher risk of major bleeding. We advise replacing aspirin with clopidogrel only 1) in PAD patients who have a vascular event despite taking aspirin (or clopidogrel in combination with aspirin in these patients) and 2) in PAD patients or who do not tolerate aspirin. In all other PAD patients prescribing low dose aspirin (75-325 mg/day) is first-line therapy.
