Abstract     Background and methods. Indications for transduodenal sphincterotomy, surgical approach to ampulla of Vateri, and immediate treatment results have been studied in 181 (4 per cent ) of all patients operated on for gall stones in an 11 year period. Stones impacted in the ampulla of Vateri, sphincter of Oddi sclerosis as well as multiple stones within bile ducts were the most requent reason for sphincterotomy. The importance of duodenotomy on the posterior wall of pars horisontalis duodeni is pointed out as well as sphincterotomy in comparison with sphincteroplasty. Results. No cases of dehiscence after duodenotomy, postoperative pancreatitis, duodenal stenosis, bleeding after sfincterotomy and treatment-related death have been observed. Conclusions. Transduodenal sphincterotomy has proved to be a safe and efficient method in the surgical treatment of some benign diseases of the bile ducts.
