Abstract     Background and methods. The author presents a surgery of problems involving parastomal hernias. Complications are stated in 10 per cent of stomal patients, more frequently in colostomy cases. For the most part the symptoms are fairly modest though more pronounced at large hernias (pain), or an acute intestinal obstruction (ileus). Efficient preventive measures will proceed from knowledge of enterostomal therapy, including a correct choice of the stoma site. Risk factors involved in the development of parastomal hernia are also: obesity, local infection, persistent caughing. Conclusions. Bowel diversion is recommended in the outer third of the rectus abdominis muscle and the left lower quadrant in colostomy. Two surgical techniques are available for the purpose: relocation of the stoma (by laparotomy or without it), or local repair without changing the stoma position. The site is frequently reinforced with a synthetic mesh. In the long range better results are obtained from relocating the stoma.
