Abstract     Background. Hemorrhagic fever with renal syndrome has been present in Slovenia continually since the year 1985, and Pomurje is the most highly endemic area. Methods and results. We show a clinical picture and therapy of hemorrhagic fever with renal syndrome. The Haantan and Puumala viruses which generally appear separately throughout Europe are widespread in Slovenia. They cause illnesses which range from asymptomatic infections to lethal concequences. In the fully developed illness we recognize 5 phases: febrile, hypotensive (shock inducing), anuretic, diuretic, convalescent. The illness ends in fatality due to shock hemorrhaging, pulmonary edema and renal failure. Variability of different clinical signs and symptoms with laboratory features for both viruses are presented. Our experiences are almost equal to the most other authors. A thorough knowledge of hemorrhagic fever with renal syndrome and therapy help consyderably toward reducing fatality. Throughout the entire course of the hemorrhagic fever with renal syndrome, fluid and electrolyte imbalance must be monitored carefully. Peritoneal dialysis or hemodyalisis considerably aid in correcting any divergence. Ribavirin given in the begining phase of the disease has proven effective. Conclusions. In the future in Slovenia in all cases of acute renal failure of infectious etiology we must take into account the hemorrhagic fever with renal syndrome. Serological confirmation of hemorrhagic fever with renal syndrome and leptospirosis is necessary because of the possibility of dual infection and probable equal course of both illnesses.
