Abstract | Peripheral neuropathy and arterial vascular insufficiency are main factors for development ulcer on foot of diabetic patient. Other factors, such as tissue trauma and infection, collaborate with this platform. Unfortunately the final result is too often amputation of toe, foot or lower extremity. At first inspection ulcer extensiveness and infection is estimated and subsequent diagnostic and therapeutic procedures are directed. Indication for inspection and treatment by surgeon is clear. Main elements of conservative therapy are ulcer debridement, regular dressings, infection therapy and off-loading of the plantar surface of the foot. Good healing is observed in smaller ulcer with these measures. A great care is necessary to prevent repeat of such complication. | |
Summary | Periferna nevropatija in motnja v arterijski prekrvavitve sta osnovna dejavnika za razvoj razjede na nogi pri sladkornem bolniku. Na to osnovo se navežejo dodatni dejavniki, kot sta poškodba tkiva in okužba. Žal je končni rezultat prepogosto amputacija prsta, stopala ali noge. Ob pregledu ocenimo obsežnost razjede in vnetja in usmerimo nadaljnje diagnostične in terapevtske ukrepe. Jasne so indikacije ko bolnik potrebuje pregled in kirurško oskrbo. V konzervativnem zdravljenju so osnovni elementi ustrezna toaleta rane, redni prevezi, zdravljenje okužbe in razbremenitev prizadetega mesta. Manjše razjede se ob tem dobro zacelijo. Nato je potrebna velika skrb za preprečitev ponovitve take komplikacije. | |
