Abstract     Background. Out of adverse reactions to drugs, only 510 per cent of them are allergic (the immunologic mechanisms are evident) and all four types of immune response are involved. Pseudoallergic reactions have similar clinical picture while immunologic mechanisms haven't been proved. In several drugs even the signs of intolerance (lowered threshold to the normal pharmacologic action) are similar to the true allergic reactions. All these reactions occur only in certain susceptible patients and are presented as drug allergies in the clinical sense. Conclusions. The most frequent allergic reactions to drugs are classified as true allergic, probably allergic and pseudoallergic reactions and drug intolerance. In the diagnostics a special attention is given to the detailed case history, skin tests, in vitro tests and incremental provocative challenge. On suspicion to allergy the principal measure is the prompt discontinuation of the therapy. If the drug cannot be replaced, in several cases the desensitization with progressively increased doses is possible. The reactions to penicilline and other beta-lactam antibiotics are discussed in detail together with the reactions to radiologic contrast media, local anesthetics and aspirin.
