Abstract     Background. Percutaneous transvenous mitral commissurotomy was first introduced by Inoue in 1982. Immediate symptomatic and hemodynamic improvement after this procedure was the reason for extending it in clinical practice. The method is currently used as an alternative to surgical mitral commissurotomy. In our institute percutaneous valvuloplasty was first performed in 1988, but it has been a routine practice in the last two years only. Purpose of the study. We aimed to present the procedure in our institution, indications and complications, patients selection and our experience with double and single balloon technique. Materials and methods. Retrograde transarterial approach with double baloon technique and single balloon - Inoue method was attempted in patients with severe mitral stenosis. Results and conclusions. Percutaneous mitral commissurotomy achieves immediate hemodynamic and clinical improvement in properly selected patients with mitral stenosis. Our conclusions are the same as described by other authors. There are very little complications. Mitral regurgitation is common, but of little hemodynamic importance. The method is an alternative to surgical mitral commissurotomy. Even if the treatment is not succesful, the patient has the same conditions for operative treatment.
