Abstract     Although labyrinthine concussion is not a rare complication of head injuries, it has rarely been reported in the medical literature. The main symptoms of labyrinthine concussion are hearing loss, tinnitus, and dizziness. The diagnosis mainly relies on audiometric tests and CT and MRI scans. The presented patient suffered a head trauma in a car accident with almost total deafness bilaterally. After the accident no sign of a fracture or brain injury was seen on CT and MRI. Patient's hearing did not improve and he was considered for cochlear implantation. On repeated MRI hypertensive lesions in the right side of the brainsteam were, which could appear after a haemorrhage and could be a contraindication for surgery. In electrically evoked auditory brainstem responses (EABR) we received good results bilaterally, so we proceeded with the cochlear implantation. Preoperative testing was confirmed with postoperative result: the patient hears well with the cochlear implant.
Summary     Čeprav je pretres labirintov relativno pogosta komplikacija poškodb glave, je malo poročil o tem problemu v medicinski literaturi. Glavni simptomi pretresa labirinta so izguba sluha, šumenje v ušesih in vrtoglavica. Diagnoza temelji na audiometričnih testih in slikovni diagnostiki (CT, MR). Predstavljamo bolnika, ki je doživel poškodbo glave v prometni nesreči s praktično gluhostjo obojestransko. Na CT in MR po poškodbi ni bilo znakov za frakturo senčnice. Stanje sluha se v času po poškodbi ni izboljšalo. V obdelavi pred kohlearno implantacijo se je postavil sum na stanje po krvavitvi v možgansko deblo, kar je bila kontraindikacija za operacijo. Z električno izvabljenimi slušnimi potenciali možganskega debla (ESPMD) pa se je dobil dober odgovor obojestransko, zaradi česar smo h kohlearni implantaciji pristopili. Predoperativno testiranje je potrdil tudi pooperativni rezultat: bolnik s kohlearnim implantom dobro sliši.
