Abstract     Background. Thromboembolisms are not only important factors in morbidity and mortality of patients subjected to extensive orthopedic operations, they are also a great social and economic problem. With appropriate physical and pharmacological protection, thromboembolisms can be reduced by two thirds and cost of treatment is reduced as well. Methods. A prospective study involving 421 high risk orthopedic patients was carried out in order to compare the effectiveness of low doses of heparin, heparin-dihydergot and ethylbiscoumacetate and to assess their cost benefit in comparison to cases when no prophylaxis was used. Results. Using all three methods thromboembolisms were diminished and an average a cost benefit of 162.3 DEM per patient was established. Ethylbiscoumacetate prophylaxis was most effective and brought the greatest cost benefit. Conclusions. Thromboprophylaxis in high risk orthopaedic patients is necessary, saves lives and reduces health care costs.
