Abstract     Background. Varicose veins and their complications are a common problem, yet there are diverging opinions as to whether radical stripping operation or incisional extraction is the primary surgical method of treatment when surgery is indicated. Methods. The aim of this study was a comparison of long term results and complications between radical stripping operation and incisional extraction of varicose veins. The duration of study was ten years (1983-1992). Major criteria for inclusion were: involvement of the long or short saphenous vein with incompetence of junctions of perforators. It was a prospective and nonrandomized trial. In the first group there were 213 patients (160 female, average 41 years, range 18-76). They were treated by radical stopping operation. In the second group there were 254 patients (240 female, average 39 years, range 17-79). They were treated on dispensary basis by incisional extraction of varicose veins. At the end of follow up results were classified as cured, when there were no symptoms and residual veins; improved when the patient was symptom free and with some residual veins; failed, when there was recurrence of symptoms and obvious veins. Results. In the first group there were 165 patients which were cured, 19 improved and 29 failures. In the second group there were 189 patients cured, 34 improved and 31 failures. The difference is statistically not significant. In the first group there were 53 patients (24,8 per cent) with saphenous nerve damage and two patients with deep vein thrombosis. In the second group there were no such complications. The difference is statistically significant. Conclusions. Incisional treatment of varicose veins has some advantages over radical stopping operation: the incidence of complications is lower and it is technically and economically justified for dispensary patients.
