Abstract     Background. Among other pulmonary diseases the diffuse interstitial lung diseases are relatively rare so they are often not recognised due to the lack of experiences. Therefore they are most frequently detected only in the progressed stages while the therapy is successful in earlier stages of disease. By this review we'd like to call attention to DILD of known etiology with relatively good prognosis as well as to the idiopathic fibrosing alveolitis with bad prognosis except in cases being detected in the earliest stage. In detection of early stages of DILD the main role is given to the general practitioner. Material and methods. One case of Farmer's lungs, one case of medicamentous alveolitis as well as one case of idiopathic fibrosing alveolitis are presented in the paper. The later was detected in the progressed stage of disease with diffuse pulmonary fibrosis and chronic cor pulmonale. By three case reports the principles of diagnostics and therapy are presented. Conclusions. By three typical cases of DILD an attention is called to the detection and recognition of early stages of DILD where in cases of known etiology after cessation of exposition a complete regression of disease is achieved, while in cases of IFA by doctrinary and lifelong therapy in early stages of disease the progression is slowed down and the survival is prolonged. Clinical signs of the early stages of DILD are: dry cough, tachypnea, dyspnea at exercise, unclear thoracic pain and often clubbing fingers.
