Abstract     Background. The shoulder impingement syndrome is presented as a loss of the normal gliding mechanism between the superior peroarticular soft tissues of the glenohumeral joint and coracoacromial arch during abduction. The combined interaction of intrinsic and extrinsic factors is presented, as well as the clinical picture, step-by-step diagnosis and treatment possibilities. Methods. From 1987 to 1992, a total of 43 patients with impingement syndrome, Neer stage II (no actual tear of the rotator cuff) and II (full-thickness cuff tear), were treated by surgery. 32 patients with a follow-up averaged 20 months (range 6 to 69 months) were evaluated on the UCLA Shoulder Rating Scale. Results. Satisfactory (good or excellent) results were achieved in 14 patients, 13 were rated fair. Four out of five poor results appeared in the group of massive cuff tears (late stage III). Conclusions. Timely opeative treatment of impingement syndrome (late stage II and early stage III) mostly results in pain relief and may prevent glenohumeral arthrosis.
