Abstract     Background. The enlarged spleen of CLL patients is irradiated when the patients are not cooperutive enough und if other methods of treatment prove unsuccessful. Methods. Twenty patients with CLL had their spleen irradiated 35 times; 14 od them received a single course of irradiation, while with the remaining 6 patients irradiation was repeated up to five times. Irradiation was given on telecobalt (60Co) or linear accelerator with a daily dose of 50-100 cGy, five times a week. Irradiation was performed for enlarged spleen and lymphocytosis in all cases. Hypersplenism accompained enlarged spleen in 19 cases. Results. The spleen size was reduced in 13/20 patients after the first course of irradiation; in 6 patients who received additional 15 courses of irradiation, a decrease was noted in 9 cases (60 per cent ). The pain ceased in ten cases but persisted in two cuses. The enlarged cervical, axillary and inguinal lymph nodes disappeared in 3 cases (23 per cent ), in two (15 per cent ) they were partly diminished, whereas in 8 cases they remaind unchanged. Total dose was 200-2075 cGy (mean 500 cGy). The period from the end of irradiation to the beginning of another treatment was 2-24 months (mean 8.7 months). There were no side effects noted. Conclusions. Apart from affecting the enlarged spleen, irradiation also induced a decrease in lymph node size, and improvement in anaemia and thrombocytopenia which resulted in Hetterquality of patients' life.
