Abstract     Background: Operative treatment of acute acromioclavicular joint dislocations is associated with high complication rates. The development of arthroscopic techniques and new implants brought forth significant advances in the treatment of these injuries. The purpose of this study was to evaluate the results of treating acute acromioclavicular joint dislocation by arthroscopic clavicle stabilization using double TightRope (Arthrex, Naples, Florida) fixation. Methods: We conducted a prospective observational study of 11 patients (ten males and one female) with type Rockwood V acromioclavicular dislocation The clavicle was stabilized by the arthroscopic technique using double TightRope(TM) to restore normal function of the conoid and trapezoid ligaments. All operations were performed by the same surgeon. At six months after surgery the results were evaluated clinically and radiologically, and assessed by the Simple Shoulder test, Constant-Murley score and subjective shoulder value. Results: The clavicle was vertically stable in all cases. Two (18 %) patients showed minor horizontal instability of the clavicle. The mean difference in the coracoclavicular distance between the operated and non-operated shoulders was 0.2mm (range 2 - 3mm). The mean Constant-Murley score was 96 (range 91-100), the mean Simple Shoulder test score was 12 and the mean subjective shoulder value was 96 (range 90-100). Conclusion: Arthroscopic treatment of acute acromioclavicular dislocation using double TightRope fixation to the coracoid process is a safe and effective method associated with good functional and cosmetic results.The period of follow up in our series was relatively short and the efficacy of reconstruction will have to be assessed on the basis of long-term results.
