Abstract     Background: Anastomotic leak is the most feared early complication following laparoscopic assisted resection of the rectosigmoid colon, responsible for a significant increase in morbidity and mortality rates in these patients. Methods and results: Between 1 January 2006 and 1 November 2009, laparoscopic assisted resection of the sigmoid colon or rectum for benign or malignant lesions was performed in 126 patients admitted to this Department. The majority were females (59.5 %) and the average age was 65.3 years. A total of 68 (53.9 %) patients were operated on for cancer, 37 (29.4 %) for precancerous lesions and 21 (16.7 %) for a benign disease. Conversion to an open procedure was necessary in nine (7.1 %) cases. In one patient, a protective ileostomy was constructed during the first procedure. Four (3.2 %) patients developed a clinically confirmed anastomotic leak, and eight (6.3 %) had minor postoperative complications. None of the patients died. Conclusion: The rate of complication rates following laparoscopic surgery for malignant or benign lesions of the rectosigmoid colon is similar to that after open procedures. It is mandatory that the operating surgeon assesses the risk for anastomotic leak and creates a preventive stoma in high-risk patients.
