Abstract     Our case presentation shows a 59-year-old female patient with systemic sclerosis, treated with d-penicillamine. During treatment, acute renal failure occured, responding well to therapy. The side effects of semicausal therapy applied in the treatment of rheumatic diseases are discussed, with special stress on gold salts, penicillamine, antimalarial drugs and sulphasalazine. The most frequent side effects of rheumatic disease treatment with cytotoxic drugs are also presented briefly. In conclusion, our position regarding the introduction of semicausal therapy and the prevention of side effects is defined, and the basic instructions for the patient are given, since this kind of therapy inevitably requires full cooperation of the patient, the competent general physician and the rheumatologist.
